Kniha Zrození kliniky je přímým pokračováním Dějin šílenství. Soustřeďuje se na dobu konce osmnáctého a počátku devatenáctého století, kdy dochází k reorganizaci medicíny, především v její výuce v klinické praxi, a kdy se medicína ustavuje jako věda. Foucault studuje dějiny struktur na různých rovinách (ekonomické, sociální, politické, ideologické či kulturní), analyzuje jejich vzájemné vztahy a v souvislosti s nimi zkoumá celkové proměny způsobů označování a vidění v dané době, hranice viditelného a vyslovitelného.

V Magrittovom maliarstve sa podľa Foucaulta dostáva do obehu obraz b ez vonkajšieho vzoru, bez imitácie niečoho daného. Takýto obraz Platón nazval simulakrom a jeho tvorcov odsúdil ako výrobcov preludov. A práve pojem simulakra nám umožňuje chápať mnohé fenomény súčasnej vizuálnej kultúry. Kaligram je teda tautológia. Ale nie taká ako v rétorike. Rétorika ťaží z nadbytku reči; využíva možnosť dvakrát povedať to isté rozličnými slovami; profituje z nadmerného bohatstva, ktoré nám dovoľuje jedným a tým istým slovom povedať dve rozličné v eci; podstatou rétoriky je alegória. Kaligram využíva tú vlastnosť písmen, že majú význam ako lineárne prvky, ktoré možno rozložiť v priestore, a zároveň ako znaky, ktoré treba rozvíjať v jednotnej reťazi zvukovej substancie. Znak, písmeno dovoľuje u rčiť slovo; čiara zasa dovoľuje stvárniť vec. Kaligram mieni teda hravo zotrieť najstaršie protiklady našej alfabetizovanej civilizácie: ukázať a pomenovať; stvárniť a povedať; reprodukovať a artikulovať; napodobniť a označiť; pozorovať a čítať.

V knize se Foucault pokouší objasnit metodologická východiska svých tří předcházejících historických studií. Kriticky reviduje užívané postupy, snaží se přesně a důsledně definovat základní pojmy (diskurs, výpověď), vymezuje se vůči tradičním disciplínám, jako je historie idejí či anglosaská analytická filosofie jazyka, a do svého repertoáru zavádí nové pojmy: diskursivní formace či nediskursivní praxe.
