Práca rezidenta na chirurgickom oddelení ikonickej Bellevueskej nemocnice je pre dvadsaťtriročného Michaela Fullera, prezývaného Mitt, splneným snom. Kráča pritom v stopách štyroch predchádzajúcich slávnych generácií Fullerovcov, ktorí v zariadení pôsobili počas jeho takmer tristoročnej histórie. Mitt ako nováčik čelí silnému tlaku, ale výhodou je preňho akýsi šiesty zmysel, vďaka ktorému dokáže vnímať aj to nehmotné.
Keď však Mittovi pacienti začnú jeden po druhom nečakane zomierať, usiluje sa tieto nevysvetliteľné úmrtia objasniť. Situácia sa mu rýchlo vymkne spod kontroly. Stáva sa obeťou vízií, v ktorých ho prenasleduje dievča v zakrvavených šatách a počuje vzdialené náreky, z ktorých mu tuhne krv v žilách. Mŕtvoly sa kopia a mladý rezident prežíva čoraz väčší stres.
