Borderline personality disorder (BPD) is estimated to have a lifetime prevalence in the United States of approximately 1.4%–2.7%. It is characterized by a long-term pattern of instability in interpersonal relationships, unstable self-image, marked impulsivity, and/or affective instability. In addition, features can be evidenced by efforts to avoid real or feared abandonment, chronic feelings of emptiness, mood reactivity, recurrent self-injurious or suicidal behavior, other impulsive behaviors with potential for self-damaging effects, intense anger or difficulty with anger control, and transient paranoid ideation or stress-related dissociative symptoms. Although these manifestations can be quite heterogeneous, the lived experience of BPD can be associated with significant emotional pain and a diminished quality of life. Many individuals with BPD have co-occurring psychiatric or somatic conditions, and all-cause mortality is increased among those with BPD. Furthermore, the lifetime burden and psychosocial impairment associated with BPD can be substantial because it typically has an early onset and can persist for many years. These significant consequences support the need for early identification of BPD. Importantly, in contrast to many earlier views on BPD, treatment is effective, and symptoms can be reduced and managed. The American Psychiatric Association Practice Guideline for the Treatment of Patients With Borderline Personality Disorder reviews current evidence and provides evidence-based statements that are intended to optimize the treatment of BPD. The guideline focuses on evidence-based treatments and includes additional information on assessment and treatment planning, which are an integral part of patient-centered care. Monografie představuje nová témata digitální medicíny spolu s návrhy jejich řešení. V průběhu roku se stala řada významných událostí – změna percepce digitalizace medicíny Ministerstvem zdravotnictví, kdy systémové změny dávají naději na realizaci tak důležitých projektů, jako je např. EHDS, i zahájení realizace projektů z Národního plánu obnovy, které jsou cíleny na obecná systémová řešení včetně legislativy, hodnocení telemedicínských projektů a posouzení využití těchto modelů v běžné klinické praxi. Za významné považuji představení projektu Telemedicína z Národního plánu obnovy, kterou předkládáme v této knize, a diskusi o implementaci EHDS včetně problematiky pacientských práv. Je velmi dobrá zpráva, že významné odborné společnosti (např. American College of Cardiology) již vydaly doporučené postupy pro některé klinicky důležité oblasti, např. pro digitální management pacientů s chronickým srdečním selháním (obr. 1, 2). Tyto postupy budou po diskusi transformovány do prostředí České republiky včetně definice úhrad plátci zdravotní péče.

Postihuje především děti a dospívající, jejichž počet každoročně roste. Pokud onemocnění postihne malé děti, jejich svět se zastaví. Dítě se cítí zrazeno nemocí, které nerozumí. Má strach. Neví, co bude, bojí se, že je to trest za něco, co udělalo špatně. Postihne-li onemocnění starší děti, především pubertální, revoltují proti nemoci, která jim bere vysněnou svobodu. Děti s cukrovkou dostanou na krk pomyslný klíč – inzulin, kterým je (eufemisticky) učíme odemykat buňky. I když se zlepšila diagnostika nemoci, máme nové druhy inzulinů, nové aplikační technologie včetně inzulinových pump a kontinuální senzory k měření glykemií, přesto to nestačí. Je potřeba vrátit dětem jejich naději a víru, že jejich dětský svět bude stejný jako dřív. K tomu potřebují pomoc především osob, které o ně pečují – rodičů, prarodičů, sourozenců, kamarádů, učitelů a prakticky celé společnosti. Jen tak lze dětem zajistit šťastné dětství a velmi dobrou metabolickou kontrolu, která jim umožní vstoupit do světa dospělých bez diabetických komplikací. Znalosti a dovednosti ne jednoduché léčby diabetu jsou základním pilířem úspěšného ovládání nemoci. A jako když otevřete čítanku a učíte dítě poznávat písmena, slabiky a slova, stejně tak se budeme společně učit základům diabetologie. Malým dětem pomohou pohádky, těm starším zvýší svobodu a možnost volby nové znalosti, především technologické.

Karcinom prostaty představuje v současné době nejčastější nádorové onemocnění v mužské populaci. Karcinom prostaty má velmi pestrou biologii chování a je to jeden z mála nádorů, který u řady nemocných díky moderní léčbě není příčinou jejich úmrtí. Tato skutečnost představuje výzvu jak pro poskytovatele léčebné péče, tak pro pacienty samotné především s ohledem na rozsah a načasování vhodné terapie. Smyslem není jen délka života, ale především jeho kvalita a to s důrazem na pacientovi preference. Problematika samotných „dermatologických katastrof“ – závažných bulózních onemocnění – se stává stále aktuálnějším tématem. Při zvyšujícím se počtu medikamentů zaváděných do klinické praxe, narůstá také pravděpodobnost rozvoje polékových dermatologických reakcí. Nejzávažnějším stavem z této poměrně heterogenní skupiny onemocnění je právě toxická epidermální nekrolýza (TEN). Jedná se o velmi vzácné onemocnění, které se manifestuje různě rozsáhlou kožní exfoliací a velmi často také slizničním postižením. Základním patofyziologickým podkladem tohoto onemocnění je indukce masivní apoptózy keratinocytů, dominantně v oblasti dermoepidermální junkce. Bohužel kompletní a uspokojivý popis patofyziologického pozadí tohoto syndromu dodnes chybí. Tato skutečnost se následně odráží na mnohdy marné terapii, což se projevuje velmi vysokou smrtností. S trochou nadsázky lze říci, že čím více o TEN víme, tím složitější se syndrom zdá být. Dodnes nebyl etablován spolehlivý terapeutický koncept, který by prokázal výraznou efektivitu vůči základnímu onemocnění, tedy zastavení propagace lokálních a systémových změn, a zároveň minimálně kompromitoval funkci imunitního systému v redukci výskytu infekčních komplikací. Kniha je zaměřena na praktické aspekty současného povědomí v oblastech, jako jsou patofyziologie, diagnostika, diferenciální diagnostika, infekční komplikace, lokální a systémová terapie zejména u pacientů s TEN. Nicméně diskutovány jsou také příbuzné syndromy ze skupiny závažných bulózních stavů. Protože za úspěšnou terapií každého z pacientů s TEN stojí vždy vysoce erudovaný a hlavně multidisciplinární tým odborníků, je kniha určena nejen pro popáleninové specialisty, kteří se o pacienty s TEN starají v České republice primárně, ale rovněž je cílena na dermatology, anesteziology, plastické chirurgy, intenzivisty a oftalmology. V neposlední řadě je kniha určena i pro ošetřovatelský zdravotní personál, neboť je stavebním kamenem v komplexní péči o pacienty s TEN.
